WHAT IS NEEDED FOR REGISTRATION?
- The child must be five (5) years of age prior to September 1, of the year they enter kindergarten.

Birth Certificate: You must provide a legal document showing the student’s legal last name. Please bring the student’s birth certificate (copy or original). If you do not have this, you can bring immigration papers or proof a guardianship.

Certificate of Immunization Status: Your doctor or the health department can assist you in obtaining this information. You may register your child prior to submitting this record, however, children may not begin school until they have completed the immunization requirements.

Proof of Address: Please provide two documents, one of which is a utility bill. Other documents may include a driver’s license, government issued identification card, voter’s registration, or a second utility bill. If you are without a place to live, please let the registrar know.

Please check our website at portangelesschools.org/families/bus-routes for updated but routes. All kindergarten children will wear a school provided bus tag the first two weeks of school. This tag includes their name, bus route, and bus stop.
Immunizations
To attend public school in Washington State, each child must have a signed certificate of Immunization Status form (CIS) showing proof of ONE of the following:
- All immunizations have been done according to state requirements (month, day, and year are required for all vaccines)
- Immunizations are in the process of being completed
- Medical exemption (with health care provider signature)
- Religious exemption (with parent/guardian signature)
- Personal exemption (with parent/guardian and health care provider signature)
Kindergarten Requirements include the following:

Hepatitis B |
3 doses (properly spaced) |

DTaP/DT |
5 doses -OR- 4 doses are acceptable only IF given on or after the 4th birthday AND at least 6 months after dose 3 |

Polio |
4 doses IF all were given before the 4th birthday -OR- 3 doses IF the last dose was given on or after the 4th birthday |

MMR |
2 doses, 1st dose must be given on or after the 1st birthday, 2nd dose must be given at least 28 days after the 1st dose |
|
Varicella |
2 doses, 1st dose must be given on or after the 1st birthday, 2nd dose must be given at least 3 months after the 1st dose -OR- A signed note from the health care provider stating the child had chickenpox (with month, day, and year) or a date of a blood test showing the child had chickenpox, or a signed exemption based on medical, religious, or personal reason. If a parent is unsure about the child’s chickenpox history, the child should be referred to their health care provider to get the vaccine. Reported history of varicella (chickenpox) is NOT accepted. |
Please make sure your child has all of the required vaccines BEFORE the first day of school.
