Register for PAHS Athletics!

Please "type" your information right in to the online forms below, then print, & deliver to Athletic Secretary Janis Bane.

Proof of ASB purchase must be provided at the time you submit athletic paperwork in order to receive a clearance card. ASB cards are purchased from ASB Bookkeeper Diana Tschimperle at PAHS. If you have any questions about fees, please her at 360.565.1531.

STEP 1:

Parent/Guardian: Complete Athletic Clearance Form annually.

Complete form (TYPE INFORMATION INTO SHADED BOXES), save and print. Athlete and parent/guardian must initial and sign form.

STEP 2:

Parent/Guardian: Complete Preparticipation Physical Exam Form annually.

Complete form (TYPE INFORMATION INTO SHADED BOXES), save and print. Athlete and parent/guardian must initial and sign form.

STEP 3:

Student's Physician: Complete Physical Examination Form every 24 months.

This form must be filled out by a physician. Make an appointment with your student's physician. Take the form with you!

STEP 4:

Parent/Guardian: Complete Inherent Risk Form for each sport student participates in - fall, winter and spring.

Student and Parent/Guardian, please read the appropriate Inherent Risk Form, sign/date.  UPDATE: Bring the signed Inherent Risk Form to Athletic Secretary Janis Bane, PAHS Main Ofice, in exchange for a clearance card.

STEP 5:

Parent/Guardian: Complete Concussion Information Form.

Student and Parent/Guardian, please read the Concussion Information Form, sign/date. UPDATE: Bring the signed Concussion Information Form to Athletic Secretary Janis Bane, PAHS Main Ofice, in exchange for a clearance card.

You're Done!

Once you've completed the forms listed in steps 1-5, return them to Athletic Secretary Janis Bane, PAHS Main Office. An athletic clearance card will be issued once all forms have been received and your student's academic eligibility is verified by the athletic department.

Athletic Eligibility

To be eligible for athletics at Port Angeles High School a student must:
1. Live within the boundaries of the Port Angeles School District and be living with your parents or your legal guardians.
2. Be in full attendance in school last semester and have passed all classes.
3. Be a full-time student currently within the Port Angeles School District and passing all classes.
4. Not be over the age of 19 or in your fifth or more year of high school.
5. Not have transferred to Port Angeles High School for athletic reasons.
6. Have a completed athletic clearance packet on file and be holding a current, valid student body card from Port Angeles High School.
7. Not be a professional athlete in the sport you are playing.
8. Abide by the current Port Angeles High School athletic code.

Seasonal Sports

Fall: Cross Country, Football, Girls Soccer, Volleyball, Boys Tennis, Girls Swimming/Diving

Winter: Boys Basketball, Girls Basketball, Gymnastics, Boys Swimming/Diving, Wrestling

Spring: Baseball, Boys Soccer, Fastpitch, Track, Golf, Girls Tennis (Try-outs for Dance Team and Cheer Team for next school year)
